Troop 3 - Forms and Documents

Several of the forms below, denoted with an "*", are fillable PDF forms into which you can directly enter your data. The completed forms can be saved on your computer, printed, or electronically transmitted.

To function properly, the forms should be downloaded to your computer (right-click the link and select the "Save Link As..." option). The fillable forms may not function correctly when opened directly in your internet browser. The forms can be opened using Adobe's Acrobat Reader or equivalent software.

Adult Forms

Adult Leaders and Volunteers - is your Youth Protection Training up to date?
- Youth Protection training is required for all BSA registered volunteers.
- Youth Protection training must be taken every two years.

If a volunteer’s Youth Protection training record is not current at the time of recharter, the volunteer will not be reregistered.
